Mandatory Password Reset In Effect

 

All ClinicalConnect users accessing the portal via https://clinicalconnect.ca are having their passwords automatically reset. Between now and December 16, 2019, check your email for a temporary password that will be sent to you, including instructions to change to a new password of your choice. For further assistance, please contact your organization’s ClinicalConnect Local Registration Authority (LRA). Additional contact information is available at https://info.clinicalconnect.ca/CC/healthcare/contact-information. You must have your supplied temporary password to use Self-Service Password Manager for this current password reset initiative.

YOU ARE ABOUT TO ENTER A SYSTEM WHICH CONTAINS PERSONAL HEALTH INFORMATION

ClinicalConnect™ is a secure, web-based portal, accessible on desktop and mobile devices that provides authorized physicians and other health care providers with real-time access to their patients’ electronic medical information (personal health information or “Patient Data”) from all acute care hospitals, Local Health Integration Networks’ Home & Community Care Services and regional cancer programs in South West Ontario, plus various provincial data repositories from which the user is authorized to view data. 

Only data contributed by these source systems will be available through ClinicalConnect, and it is possible that not every patient will have Patient Data in ClinicalConnect. 

The Personal Health Information Protection Act, 2004 (PHIPA) and the ClinicalConnect Terms and Conditions permit access to Patient Data through ClinicalConnect for the sole purpose of providing or assisting in the provision of health care to the patient to whom the Patient Data relates. PHIPA and the ClinicalConnect Terms and Conditions do not permit users to access their own or a family member’s Patient Data through ClinicalConnect. PHIPA and the ClinicalConnect Terms and Conditions also do not permit users to access Patient Data through ClinicalConnect for research or any other secondary purpose. Any unauthorized access to Patient Data through ClinicalConnect can result in disciplinary action that may include disabling of user accounts, termination of working relationships, reports to health regulatory colleges, and/or to the Information and Privacy Commissioner of Ontario (IPC/O). Legal action can also be taken against individuals in addition to being fined under the provisions in PHIPA. By logging into 
ClinicalConnect, you agree to the terms set out in the User Agreementand agree to comply with your obligations under PHIPA.
